Carroll Hospital Center's Outpatient Nutrition Counseling Service team of registered dietitians (R.D.) offers easy-to-follow, practical food and nutrition information that promotes and teaches individuals how to make healthy dietary choices and improve overall well-being.
How to Obtain Outpatient Nutrition Counseling Services
1. Ask your physician to write a referral form for our services.
2. Fax your referral form to our scheduling department at 410-840-4016.
3. Once we have received your referral, we will contact you to set up an appointment.
