If left untreated, the disease can lead to serious health complications, such as heart disease, stroke, blindness, kidney disease and nerve damage. Diabetes Self-Management Education Program
The cornerstone of our educational programs, this course helps patients gain a more detailed understanding of their condition and learn important self-care skills—the keys to better diabetes management and prevention of debilitating side effects. Offered privately and in a group setting, the program is led by a registered nurse and registered dietitian, both of whom have advanced training in diabetes care and education.
One-on-One Instruction
For patients who need more personalized education, our Diabetes Center offers a one-on-one education course that includes an introductory two-hour session and several follow-up sessions (the number of which may vary depending on insurance allowances). As part of the course, patients will:
- Learn more about their disease, including the different types and causes of diabetes
- Review the best strategies to prevent serious, long-term complications
- Master important self-care skills, such as blood glucose monitoring, medication management, foot and skin care and stress-reduction techniques
- Receive meal planning and nutrition guidance based on their unique needs and preferences
- Set measurable goals designed to improve their quality of life
- Receive regular feedback on their progress
Living with Diabetes Group Classes
Our group classes offer a comfortable, supportive setting where patients can learn more about their condition and gain the guidance and motivation necessary to make important lifestyle changes. Group classes include:
- An initial one-hour, private assessment that includes a brief physical exam, instruction for using a blood glucose monitor and receipt of a food diary to complete prior to the first group session
- Four, two-hour group sessions in which participants learn detailed information about diabetes, nutrition, self-care and prevention of long-term complications
- An individual follow-up session three months after completion of the class to assess progress and reinforce the diabetes management strategies that were reviewed
Wound Care ServicesPatients who suffer from chronic, non-healing wounds caused by diabetes or other conditions can benefit from state-of-the-art care at the
Center for Wound Care and Hyperbaric Medicine, conveniently located in the hospital’s Outpatient Center. Staffed by a specially trained, multidisciplinary medical team that includes general and vascular surgeons, nurses and educators, the Center offers the most advanced treatments available, including hyperbaric oxygen therapy, which helps wounds heal more quickly.
Gestational Diabetes Management
For women who develop gestational diabetes during pregnancy, careful monitoring is important to ensure the health of mother and baby. At Carroll Hospital Center, all patients diagnosed with gestational diabetes receive an initial, two-hour consultation at the Diabetes Center, in addition to follow-up sessions on an as-needed basis. These sessions teach expectant moms how to manage their nutrition, exercise and regular glucose monitoring to effectively control their diabetes. Instruction for insulin dosing and injections also is provided, if appropriate.
