During the event:
• Talk one-on-one with an outpatient palliative care nurse
• Receive education on the importance of advance care planning
• Receive help with completing an Advance Directive (health care power of attorney, living will)
• Have your Advance Directive form witnessed and placed on file at the hospital
• Bring your current health care decisions documents to be filed at Carroll Hospital

Appointments are recommended, but not required. Walk-ins are welcome.
